Communication between you and our office is of key importance to us. In addition to the expert professional medical care we provide, we are equally proud of our efficient and courteous support staff. We want your office experience to reflect our friendly atmosphere and our attention to you and your individual needs and concerns.

Your Appointment

We respect your time and make every effort to see you at your scheduled time. We ask that you are on time for your appointment to prevent impacting others scheduled after you. This is especially important if you are scheduled for a procedure. Procedure appointments require a longer amount of time and are most difficult to “work in” as they also require their full scheduled time allotment. If you are late for your appointment, we may need to reschedule the appointment.

We request 24 hours notice if you need to cancel or reschedule an appointment. This allows us to use the time for other patients. We do have a charge of $25.00 for the second time you miss an appointment, without notifying the office.

Treatment of Minors

At their initial appointment a minor child must have a parent/guardian signature. Additional signatures are also required at each subsequent visit if a treatment/procedure is scheduled.

Insurance and Payment

Certain preventative services are available to patients without co-payments, co-insurance or deductibles.  Unfortunately, skin cancer screening exams are not included in those preventative services by the United States Preventative Services Task Force (USPSTF).

We highly recommend regular exams to detect early changes with your skin, as skin cancer can be disfiguring and in certain cases can lead to death.

Please be aware that skin cancer exams in our office will be billed to your insurance with evaluation codes and not as a preventative service.  Patients will be subject to co-payments, co-insurance and/or deductibles per their insurance contract.

Cosmetic services or those services considered not medically necessary by insurance companies are payable at the time of service. We participate with most major insurance plans and traditional Medicare. Please note that we do not participate in any of the Medicare Advantage plans (examples: Medicare Senior, Medicare Advantage, Medicare Fee for Service). Our staff will provide you with participation information for your particular plan if needed. In order to bill your insurance, we require your insurance card at your initial visit, or when your insurance changes. If your plan requires a referral, it is your responsibility to ensure that the referral is available at the time of your appointment. Co-pays and deductibles are due at the time of service.

You can reach us by phone at (937) 436-1117 at any time. After hours voice mail is available in order to cancel/reschedule appointments, request prescription refills and in case of urgent needs. For true emergencies call 911. In the case of urgent needs, our voice mail will instruct you to leave a message. These messages are typically responded to in less than two hours.
